It’s easy for DC Residents to make an appointment online. Simply select an available date, time and pay a $25 registration fee. Once registered, you will receive an appointment confirmation with the location of your doctor’s visit. A $125 cash co-pay is required at the time of your consultation. The application fee for the Department of Health is an additional $100 money order ($25 for qualifying low income patients) made payable to “DC Treasurer.” Please see the checklist in the “Forms” section for list of items to bring with you and if you qualify for low income. At your appointment you will be examined by a physician who will assess your medical history and current medical condition. If qualified, your physician will send a recommendation to the DC Department of Health. After the recommendation is submitted, you will receive an email confirming receipt. No further action is required on your part.

Completed applications are then submitted to the DC Department of Health. Once your application is processed and approved, you should receive your card by mail within two to three weeks.
